Skincare advice from my derm
If there’s one thing I’ve learned in over ten years of dealing with acne, it’s that sometimes, over the counter products just aren’t enough. Frustratingly, the healthcare system in the US makes dermatologist visits and prescription meds very expensive. That’s why I want to share some of the advice I’ve gotten from my dermatologist. While I do use several prescription medicines to treat my acne, the tips and tricks I’ve gotten from my derm are useful for anyone dealing with the same issues as me (hormonal acne, as well as the texture and hyperpigmentation left behind). For reference, my skin is not sensitive—not sure if I was born that way or if I’ve built up a tolerance to harsh products over the years. I use a telemedicine service called Apostrophe, and before that, I used Curology. You fill out a questionnaire on your medical history and skin concerns, upload pictures of your skin, and a dermatologist reviews the information and creates a treatment plan for you. I have a whole post on why I switched from Curology to Apostrophe, but the short answer is that Apostrophe prescribes more oral medications and I wanted to try that route. I’ve had an excellent experience with both companies in terms of the detailed advice I’ve gotten from my providers. So let’s get into it! Here are some things my dermatologists have told me: Don’t pick at your skin... seriously. One of the best tips I got was to use hydrocolloid patches on acne with a head: not only does it help the bump go down way quicker, it keeps me from picking at it any further. I LOVE the COSRX - Cosrx Acne Pimple Master Patch and I never go without them. Look at your diet. My derm says she gives this advice to ALL her patients: limit sugar, processed carbohydrates, and dairy. She says food is not generally the main cause of acne, but it can certainly aggravate it. She also always recommends increasing consumption of fruits and vegetables. Use sun protection—it helps dark spots fade faster, and of course prevents wrinkles. My derm recommends wearing SPF 30 or higher, as well as a wide-brimmed hat when outdoors. The days of minimal skincare for men are over! The global market for skincare is expected to reach $189 billion globally by 2025 (statista.com). These companies are no longer missing out on the huge market that is expanded through men of all ages. Sisinternational Research also did studies that show men have a few preferences when it comes to skincare: easy to use products, leave no residue, and no strong scent. So, men's specific skincare products are ultimately completely unnecessary. They do have simple packaging and added cologne fragrances, but the ingredients do not differ in any positive way. Physiological differences between men and women's skin usually shows through larger pores, more oil production, and thicker overall skin. As I said before, it used to be seen as a feminine endeavor to go down the skincare aisle. Through more education and gender neutral marketing, people are picking up cleansers and moisturizers without the stigma. So, put down the 3-in-1 shampoo and face wash combo and try these suggestions out instead. CeraVe - Foaming Facial Cleanser For Normal To Oily Skin Great budget friendly option for most skin types. Using a foaming but non stripping wash can help keep it clean and hydrated. Krave - Kale-Lalu-yAHA Gentle exfoliant that works great to ease your way into a skincare routine. Use 1-2 weekly and increase over time. This can help with pesky ingrown hairs and irriation caused by facial hair. Do not use same day as shaving! dermalogica - Daily Microfoliant Exfoliator Physical exfoliant option if you prefer a scrub feeling that can be used before shaving. Use 2-3 weekly and assess skin's response. Only choose one exfoliant to use at a time. OLE HENRIKSEN - Truth Serum Optional vitamin C is a great way to brighten up and even the skin tone. Use every morning before SPF. Leaves no sticky feeling and sinks in nicely. LA ROCHE-POSAY - Toleriane Double Repair Face Moisturizer UV SPF 30 Sunscreen is an absolute must for everyone! This packaging is clean and neutral, and a squeeze tube makes it easy to store in a shaving kit. Be sure to apply quite a bit and reapply throughout the day. Working at the spa, I got quite a few male clients for facials. Taking care of your skin at home or professionally is necessary no matter what. Do not be afraid to purchase the essentials: cleanser, exfoliant and SPF. After a while of getting your routine down, then you can add in extra products as needed for your specific skin type. Every once in a while, it is important to do a deep cleanse for your skin to prevent sebum and dead skin build up that will lead to break outs and enlarged pores. It is like doing house cleaning, which is necessary for everyone regardless of your skin type. However, since my face produces a lot of oil, I tend to break out more, and I have developed a deep cleanse routine that really helped with alleviating my acne problems.   Step 1 – Cleanse Product: Neutrogena (Deep Clean Facial Cleanser Normal to Oily Skin)    Firstly, I need to get rid of all the dirt and impurities on the surface of my face to get into cleaning the deeper layers of my skin. I use this cleanser because it has a strong cleaning power that removes the oiliness of my face without stripping off the hydration. It is in a gel texture that lathers into foam.      Step 2 – Exfoliate Product: Dermalogica (Daily Microfoliant)   This step is to get rid of all the dead skin cells being built up on the face that are the main causes of rough skin, dullness and acnes. The product has become one of my favourite products because my skin feels much brighter and smoother every time after I have used it. It is in a white powder form and turns into foam when it touches water. Because it is meant to be a daily exfoliate, it is very gentle on the face, however, I would not recommend exfoliating every day because it will destroy your skin’s barrier, and I have noticed that my skin becomes more fragile and red if I use it too often.   Step 3 – Detox Product: Estée Lauder (NightWear Plus 3 Minute Detox Mask)   I always like to follow up with a clay mask that draws out impurities from deep under the skin and helps control the oiliness of my face. This visibly reduces black heads of my skin if I keep up with this routine regularly. This product smells like fresh cucumber, which I enjoy, and has a very cooling sensation on the face. What I like the most about this product compared to other clay masks is that it dries very quickly because it is so thin. It leaves me with a clearer and smoother skin just after 3 minutes, which is great for lazy people like me.   Step 4 – Sooth & Hydrate Product: Avene (Eau Thermale Spring Water)   I use this mist to add another layer of moisture before I lock everything in with a moisturiser and to sooth my skin after exfoliation. The mist spreads very evenly, covering every part of my face, and feels very refreshing and cooling.     Step 5 – Moisturiser Product: La Roche Posay (Toleriane Sensitive Crème)   Lastly, I will rehydrate myself with a thicker cream than I normally use that has soothing properties to comfort my skin after going through the hash cleansing steps before. My skin feels very hydrated, smooth and plump after I use this cream. Also, it is not too heavy on the skin, and has no scent.

As skin care companies develop new products to improve our skin, it becomes more complicated and confusing as to how to apply all these products and get the best results.  If they are not layered properly, you won’t get the benefits and you will have wasted your money.  As a rule, products get layered from thinnest or lightest feeling to thickest. Morning routine: Cleanser - Always use warm water and pat dry. Never rub.  I like LA ROCHE-POSAY - Toleriane Hydrating Gentle Face Cleanser or Cetaphil - Gentle Skin Cleanser Toner - You may not require one depending on the pH balance of your cleanser.  I do not personally use one. Essence - This helps to hydrate and nourish your skin.  I have patients that love SK-II - Facial Treatment Essence (Pitera Essence) Anti-oxidant serum - My fav is SKINCEUTICALS - Phloretin Cf With Ferulic Acid Eye cream - This is for everyone, but this step is especially helpful for patients with dark circles.  Pat the product gently into the delicate skin. Never rub.  Skinceuticals  makes a great eye cream called SKINCEUTICALS - A.G.E. Eye Complex for Dark CirclesAcne treatment Moisturizer - I like SKINCEUTICALS - Emollienceas well as Cetaphil - Cetaphil Moisturizing Cream Physical  or Mineral Sunscreen - If you use a chemical sunscreen, it goes on before your moisturizer.  My fav chemical sunscreen is LA ROCHE-POSAY - Melt-in Sunscreen Milk SPF 60, and one of my fav physical sunscreen is TIZO - Mineral Sunscreen for Face SPF 40 Evening Routine: Oil cleanser to remove makeup -Always use warm water and pat dry. Never rub. I have used Neutrogena - Ultra Light Cleansing Oil Cleanser - Always use warm water and pat dry. Never rub. Toner - You may not require one depending on the pH balance of your cleanser. Essence - This helps to hydrate and nourish your skin. Acne treatment, exfoliator, acid or retinoid for anti-aging - I won’t live without my retinoids.  A great over-the-counter one is DIFFERIN - DIFFERIN Acne Treatment Gel Serum - Such as those that contain peptides and growth factors.  One of my fav is SkinMedica - Tns Essential Serum Eye cream- This is for everyone, but this step is especially helpful for patient with dark circles.  Pat the product gently into the delicate skin. Never rub. Thicker Moisturizer - If you have dehydrated skin, otherwise you can use your morning moisturizer. I like LA ROCHE-POSAY - Toleriane Double Repair Face Moisturizer UV SPF 30 Always remember to give your skin time to absorb the products.  Do not layer them one right after the other.  Always listen to your skin because it will tell you what it needs.  If it is extra dry, you may not want to use a toner, exfoliator or acid that night.  If you have any questions, always consult with your dermatologist.

A year ago, I started being lazy and was not following my skin care routine. To give a little background about me, I’m a nurse who does shift work, so on days where I come off from a night shift, I would shower and only rinsed my face with water without using a cleanser. I then would apply my night cream and go to sleep. Fast forward, I started developing this tiny little bumps all over my face, occasional break out, hyperpigmentation and uneven texture. I would exfoliate at times but became inconsistent with it so It didn’t really clear up my skin. Back in March of this year, I became more frustrated as it was taking longer for me to clear up my skin and these tiny little bumps kept coming back (no they were not fungal acne as they were bigger in size and reddened)so I decided to change up my skin care products and made up a routine similar to the ones I see on my social media platform. I use different kinds . HADALABO - Gokujyun Premium Lotion or COSRX - Hyaluronic Acid Hydra Power Essence - Moisturizer if I feel like my face needs extra hydration otherwise I would skip this part CeraVe - Moisturizing Cream For Normal To Dry Skin or LA ROCHE-POSAY - Lipikar Balm AP + Body Cream - SUNSCREEN!! LA ROCHE-POSAY - Anthelios 50 Mineral Ultra Light Sunscreen Fluid SPF 50 or LA ROCHE-POSAY - Toleriane Double Repair Face Moisturizer UV SPF 30 NIGHT ROUTINE - Double Cleanse heimish - Heimish All Clean Balm or The Ordinary - Squalane Cleanser followed by CeraVe - Hydrating Face Cleanser For Normal To Dry Skin - Toner COSRX - AHA/BHA Clarifying Treatment Toner or PAULA'S CHOICE - Skin Perfecting 2% BHA Liquid Exfoliator - If I am not using any of the toners mentioned above, then I would use this The Ordinary - Lactic Acid 10% + HA On the nights I use retinol, I don’t use any chemical exfoliants at all PAULA'S CHOICE - Clinical 1% Retinol Treatment, 30ml - Moisturizer CeraVe - Moisturizing Cream For Normal To Dry Skin -Facial Oil The Ordinary - 100% Plant-Derived Squalane or DERMA•E - Rejuvenating Sage & Lavender Face Oil This was my routine for 4 months. Hello all! This is my skin care journey. I have always had acne. For as long as I can remember, probably since I was roughly in 5th grade (I am 22 y/o now) and I have always struggled. It always seemed no matter how many products I tried, methods I used, nothing would help. I finally found one product that helped more than others; La Roche Posay Effaclar wash. This product helped me get even SEMI better skin. However, it just didn’t cut it. I finally broke and scheduled an appointment with a dermatologist in the company I work for. We tried antibiotics, creams, gels, prescription face washes, and literally anything you can think of, I have tried at least once. My derm finally got to the point where he decided Accutane would be the best route for me. I completed 5 months of Accutane, and the change in my skin and my confidence in my skin changed MY LIFE. If you struggle with acne like myself, you know the weight acne puts on you, and how it makes you feel absolutely worthless. Acne is a struggle, and it can definitely cause anxiety, and even depression. Finding a product that changes your life is mind blowing. I have been off of Accutane around 7 months now. The oiliness in my skin is starting to pick back up, I can tell my pores aren’t nearly as tiny as they were on Accutane. I’ve even started developing itty bitty zits, and black heads on my chin, nose, and T zone. I reached out to my dermatologist and he recommended a benzoyl peroxide face wash, and differin (adapalene) gel nightly. I’ve been doing this for about a month, and I can already see a difference in my skin AGAIN. Pores are smaller, oiliness is decreased immensely, and LESS ACNE. However, I have an insanely bad habit of picking at my face, and I tend to pick at tiny little things that I can hardly see, and others can’t see at all. Then I cause irritation to my skin, and make something that wasn’t even a problem, a big problem. I’m still working on not picking, but it gets the best of me, as I’m sure it does most of you. I’m always available and willing to help, and answer any questions anyone may have about skin care, and the best products I’ve found along the way that are staples in my routines.
